Self Refer

Self referrals can be made via the contact me form on this website, please ensure to include your phone number and best time to call you to discuss your needs further, this will ensure your referral is triaged and the first available appointment time can be allocated quickly.   If you already have a Mental Health Treatment Plan, please feel free to send it through to me directly as an attachment to nicola@southperththerapy.com   

Workplace/HR Department Referral

For all workplace/HR referrals please send through employee details including name, date of birth, address, contact number and best time to call, please ensure to provide some brief background info that supports the referral and confirmation of how many sessions are required.  Email to;   nicola@southperththerapy.com

Parent/Guardian Referral

If referring a young person, please note that generally those over the age of 16 will be seen individually.  Please include your own details and relationship to the young person, as well as their name, date of birth and brief background info via the 'contact me' form or via email at nicola@southperththerapy.com
