Self Refer

​Self referrals can be made via the 'contact me' form on this website, please ensure to include your mobile phone number and email address, this will ensure your referral is triaged and the first available appointment time can be allocated quickly.  If you already have a Mental Health Treatment Plan, please feel free to send it through to me directly as an attachment to nicola@eastperththerapy.com   

Workplace/HR Department Referral

For all workplace/HR referrals please email nicola@eastperththerapy.com with some brief background info that supports the referral and confirmation of how many sessions are required. 

Parent/Family Member Referral

If referring an adult family member please include your own details, as well as the name of the person you are referring and their mobile phone number via the 'contact me' form or via email at nicola@eastperththerapy.com

Please note that the clinic only provides assessment and treatment to adults over the age of 18 and does not offer services to children.
